return materials when they are due

report loss or theft of card immediately

($3.00 may be charged for a lost or stolen card)

pay any fines or fees charged against a library card before it was reported lost or stolen

report any changes in home address, e-mail address or telephone number to the Library

not loan the card to anyone, including a family member

be responsible for all fines, damages, losses and collection costs (including attorney fees) charged against the card

I am responsible for monitoring this minor's use of all library resources

I may not check out restricted materials on a minor's restricted card
